PUBLIC SERVICE PROVIDER SURVEY

The City of Mentor is preparing its 2022-2026 Consolidated Plan and its Analysis of Impediments to Fair Housing study. An important part of the Consolidated Plan and Analysis of Impediments includes input from Public Service Providers. This survey has been created to capture your insights into specific populations, current conditions, existing resources, and gaps in services, which will guide the City in how to best utilize its CDBG funds over the next five years.

The City of Mentor receives approximately $160,000 each year from HUD and invests in eligible activities, including: housing, economic development, public facilities, social services, and planning. All funded activities must meet one of the following national objectives: Assist low and moderate-income households; eliminate slum or blight; or respond to an urgent need.

All feedback will be reviewed to generate a compilation of qualitative and quantitative data that can be integrated into the City’s 2022-2026 Consolidated Plan and Analysis of Impediments study.
