The Lake County General Health District (LCGHD), in conjunction with University Hospitals, is currently conducting the 2022 Lake County Community Health Needs Assessment.
The community resident survey, the public component of the assessment, is currently available for Lake County residents years ages 18 years of age and older to complete. The community resident survey covers a variety of topics ranging from personal health, wellness, health care utilization, and health behavior. The survey is available in both English and Spanish, and can be accessed by visiting or www.lcghd.org, or by the following direct links https://bit.ly/LakeCHNA2022Eng (English) and https://bit.ly/LakeCHNA2022Esp (Spanish). The survey takes less than 15 minutes to complete, and is both voluntary and confidential. As a thank you, those who complete the survey can choose to be entered into a raffle to win one of ten $50 VISA gift cards. The survey is available until at least February 22, 2022, and is compatible with PC, Mac, tablet, and mobile devices.
The purpose of the assessment is to understand the current health needs of Lake County residents, as well as identify both where and how the health of the community can be improved. The assessment includes a review of national and state data sources, the community resident survey, and targeted community focus groups. This year’s survey will also gather information on the impact of the COVID-19 pandemic, as well as further explore the social determinants of health.
“The population’s health is divided by the social determinants of health resulting in vastly different qualities of life. The resource-rich and well-connected are not only are far more likely to receive care when they need it, but they also start off with advantages that have a tremendous effect on health — in housing, employment, stress levels, food security, social capital and more,” said Health Commissioner, Ron H. Graham. “Health starts in our homes, schools, workplaces, neighborhoods, and communities. We must partner to create social and physical environments that promote good health for all.”
