Deer Harvest Report Form

If you harvest a deer it is your duty to abide by the State of Ohio regulations on game checking as well as report through this form, or return the City of Mentor deer harvest form to the City of Mentor Police Department within 24 hours. Please complete all fields in their entirety.

If you have any questions about reporting your deer or archery hunting in the City of Mentor, the Mentor Natural Resources Division can be reached at (440) 974-5717 Monday through Friday from 8 AM – 5 PM. If you have an emergency regarding your harvest and we cannot be reached, please contact the Mentor Police Department non-emergency dispatch at (440) 255-1234.
