The Lake County General Health District (LCGHD) is now scheduling dedicated COVID-19 vaccine clinics for children 5 to 11 at 5966 Heisley Road in Mentor, Ohio. Due to the limited supply of vaccine received, vaccines will be given by appointment only. LCGHD will only be vaccinating children 5 to 11 years old at these clinics. No walk-ins will be accepted. Parents may schedule an appointment at 440-350-2188, or through ArmorVax (https://armorvax-app.azurewebsites.net/Account/Login).
A parent or guardian must be present with the child and sign a consent form. Parents are also asked to bring their photo identification and insurance cards if able. Clinic dates and times are listed below:
- November 13thfrom 1:00 p.m. to 3:00 p.m.
- November 16thfrom 2:00 p.m. to 6:00 p.m.
- November 18thfrom 1:30 p.m. to 4:00 p.m.
- November 19thfrom 9:00 a.m. to 4:00 p.m.
- November 23rdfrom 9:00 a.m. to 6:00 p.m.
- November 30thfrom 9:00 a.m. to 12:00 p.m.
