Sealed proposals will be received at the Purchasing Department, City of Mentor, 8500 Civic Center Boulevard, Mentor, Ohio until 12 noon on July 17, 2020 and will be opened and read immediately thereafter for the following roadway resurfacing project:

Bids must be in accordance with specifications and on forms provided below or RFP’s will be available for pick-up at the Purchasing Office for the cost of printing. Bidders shall be responsible for checking for Addenda and obtaining any from the website.
Only those contractors which are prequalified for work by ODOT pursuant to Section 102.01 of the CMS will be eligible to submit bids for the construction of this project. Each bid must be accompanied by a certified or cashier’s check in the amount of 10% of the amount bid, an irrevocable letter of credit in the amount of 10% of the amount bid or an original bond in the amount of 100% of the amount bid per ORC 153.54 and 153.571.
Disadvantaged Business Enterprise (DBE) Requirement. DBE participation goals (subcontracts, materials, supplies) have been set on this project for those certified as DBEs pursuant to Title 23, U.S.C. section 140(c) and 49 CFR, Part 26, and where applicable qualified to bid with ODOT under Chapter 5525 of the ORC. The DBE Goal for this project is 8%.
Record Retention: As the LPA, ODOT or the United States Government may legitimately request from time to time, the Contractor agrees to make available for inspection and/or reproduction by the LPA, ODOT or the United States Government, all records, books, and documents of every kind and description that relate to this contract.
