The Lake County General Health District (LCGHD) and Lake Health are partnering to conduct a comprehensive health assessment of the Lake County community.

The public component of the community health assessment will begin today with the release of the community resident survey, which can be completed by Lake County residents 18 years of age and older.

The purpose of the assessment is to understand the current health needs of Lake County residents, as well as identify both where and how the health of the community can be improved.  The assessment includes a review of national and state data sources, in conjunction with distribution of the community resident survey.

The community resident survey covers a variety of topics ranging from personal health, wellness, health care utilization, and health behavior.  Responses will provide the county with vital information about the health status of the community.  As a thank you, those who complete the survey can choose to be entered into a raffle to win one of ten $100 VISA gift cards.

“This is the first time we are bringing community partners together in the county to collaborate on a single assessment of the health care needs in our area,” said Dino DiSanto, vice president of Lake Health Marketing and Government Affairs.  “It’s a true partnership that goes beyond just Lake Health and the Health District.  It’s a model for other communities to follow who want to do a similar health assessment”.

Hard copies of the English and Spanish language surveys will be available at specific locations for those who may not have access to the electronic survey.  For those interested in completing the survey online, both surveys will be available for 30 days from the time of issue, and are compatible with PC, Mac, tablet, and mobile devices.

“The heart of this community health assessment will be a layered diagnosis of the health problems faced by Lake County residents, and the day-to-day context surrounding those problems,” said LCGHD Health Commissioner Ron Graham.  “As health is multifaceted, so is our assessment approach, and the collaboration with Lake Health is key”.
