On Wednesday, August 13, 2025, at approximately 3:30 PM, officers from the Mentor Police Department responded to a multiple vehicle accident on I-90 eastbound between SR-306 and SR-615.

Initial investigation revealed that a 2018 Jeep Lattitude rear ended a 2025 Toyota RAV4, which his caused the RAV4 to strike another vehicle.

The Jeep continued and struck two additional vehicles;  a trailer being pulled by a semi truck, and a 2017 Ford Focus.

The driver, and only occupant of the Jeep, James Kephart, 85, of Geneva, Ohio, was pulled from the vehicle by two witnesses and transported to Mentor Cleveland Clinic Hospital by the Mentor Fire Department where he was pronounced dead.

The two occupants of the RAV4 were transported to UH Lake West Hospital in Willoughby with non-life-threatening injuries.

The roadway was closed for approximately two and a half hours. The investigation is ongoing and no additional information is available at this time.
