The Ohio Department of Aging (AGE) is in the process of developing Ohio’s 2027-2030 State Plan on Aging (SPOA). The State Plan on Aging is a comprehensive roadmap to improve the overall health and well-being of older Ohioans.

To help achieve their vision of making Ohio the best place to age in the nation, they are asking as many Ohioans as possible to take the department’s Statewide Needs Assessment Survey to enable the department to better understand what matters most to older Ohioans, adults with disabilities, and their caregivers. Feedback from those digital surveys will be used to further bolster the 2027-2030 State Plan on Aging. This survey is designed to allow you to answer anonymously and consists of easy-to-answer multiple-choice questions. It should take 10-15 minutes to complete.
