Sealed proposals will be received at the Purchasing Department, City of Mentor, 8500 Civic
Center Boulevard, Mentor, Ohio until 12 noon on March 25, 2016 and will be opened and read
immediately thereafter for the following roadway resurfacing project:
LAK – 306 – 5.53 RESURFACING
OPINION OF PROBABLE CONSTRUCTION COST: $ 3,750,000
COMPLETION DATE: 10/30/2016
Bids must be in accordance with specifications and on forms provided on website or packets will be available for pick-up at the Purchasing Office Monday through Friday between the hours of 8 AM and 5 PM for the cost of printing. Bidders shall be responsible for checking for Addenda and obtaining any from the website.
Only those contractors which are prequalified for work by ODOT pursuant to Section 102.01 of the
CMS will be eligible to submit bids for the construction of this project. Each bid must be
accompanied by a certified or cashier’s check in the amount of 10% of the amount bid, an
irrevocable letter of credit in the amount of 10% of the amount bid or an original bond in the
amount of 100% of the amount bid per ORC 153.54 and 153.571.
DBE Participation goals (Subcontracts, materials, supplies) have been set on this project for those
certified as DBE’s in accordance with MAP-21 (2012) and 49 CFR, Part 26, and qualified to bid with
ODOT under Chapter 5525 of the Ohio Revised Code. The DBE Goal for this project is 5% .
