Description of the service
The Lock Box Program enables Fire/EMS/Police personnel to gain access to your home in the event that you cannot reach your door to open it for them during an emergency. The program provides a secure Lock Box that can only be accessed by Fire/EMS/Police personnel by using a special key. No tools or drilling is required for installation. Simply hang it on your door.
Can I choose to sign up for more than one of these programs?
Yes. You can participate in only the Lock Box Program, or you can sign up for multiple programs.
Is there a cost?
There is a one-time fee of $35.00.
How to sign up
You must sign up in person for this service at Mentor Fire Department Headquarters located at 8467 Civic Center Blvd, Monday through Friday, 8:30 AM to 4:30 PM. You can complete the Lock Box Waiver Form in advance.

Description of the service
A notation or “flag” is placed in our Computer Aided 9-1-1 Dispatch System (CAD) to make Mentor Police, Fire and Ambulance aware of any special medical needs, emergency contact information and/or lock box/hidden key information you would like them to be aware of if they are called to your residence in an emergency.

For residents of Mentor who need electricity for medical needs (i.e. oxygen) and/or for people who are living alone, elderly or disabled and do not have any family or friends to check on them during a large scale emergency or long term power outage
Description of the service
Includes everything the Resident Information Address Flag includes, plus the addition of an extra layer of service to qualifying residents. In the event of a long term power outage or other large scale emergency, enrolled residents will be checked on by emergency services personnel by phone first, then in person if unable to be reached by phone. This is not a daily check in service.
Please Note – The Well Check Database automatically includes the Residential Information Flag so there is no need to sign up for both programs.

Any Ohio resident age 60+ who would like to be checked on with a daily phone call.
Description of the service
The Program will provide a daily call, during the same one-hour time period every day, to a qualified Participant in the Program. These will be automated calls. If no one answers the call, the Program will call the Alternate Contact Person, if one has been selected by Participant. If the Program is unable to contact the Participant, or leave a message with Participant’s Alternate Contact Person, the Program will notify Participant’s local non-emergency service.
Can I choose to sign up for more than one of these programs?
Yes. You can choose to participate in only the Ohio Staying Connected Program or you can choose to sign up for any of the City of Mentor Programs along with it.

Lifeline, Inc. has launched a new Are You OK Program, in partnership with the Board of Lake County Commissioners. The Are You OK Program is a safety assurance program for Lake County senior citizens (60 and older) who may be isolated or living alone. Seniors can enroll in the program to receive daily automated monitoring calls designed to provide security and ease the concerns that friends and families may have for their loved ones’ safety.
Through the program, which will be operated by Lifeline’s 211 service, each enrolled senior will receive several automated calls at pre-arranged times each day. If those calls are not answered after three attempts, a safety alert is sent to Lifeline’s 211. Lifeline’s 211 team will contact emergency contacts and, if necessary, local law enforcement for well-check visits. Seniors who enroll in the program will be required to have a lock box installed on their home through their local fire department—Lifeline’s 211 can provide information on how to obtain a lock box.
Senior citizens wishing to enroll in Are You OK can call Lifeline program coordinator Andrea Johnson at 440-354-2148 ext. 207 or email at andreaj@lclifeline.org to see if they qualify and enroll. Program participants will have to meet age and residency criteria and agree to have a lock box installed on their home.
